I've never used it but you might be able to use a directory print utility such as:
JR Directory Printer
http://www.spadixbd.com/freetools/jdirprint.htm
And then print the contents of your start menu folders:
C:\Documents and Settings\All Users\Start Menu
C:\Documents and Settings\User Name\Start Menu
